Indications/Uses
Dosage/Direction for Use
Initially 5 mg once daily. Max dose: 10 mg once daily after at least 2 wk. Maintenance dose: 5-10 mg once daily.
Administration
May be taken with or without food: May be taken w/ a light meal not rich in carbohydrates/fat. Swallow whole, do not divide/crush/chew.
Contraindications
Hypersensitivity. Cardiogenic shock; severe aortic or mitral stenosis; obstructive hypertrophic cardiomyopathy; unstable angina pectoris; acute MI w/in 4-8 wk; decompensated heart failure. Severe hepatic impairment. Pregnancy & lactation. Childn.
Special Precautions
Patients w/ conduction disorders, compensated heart failure, tachycardia, aortic &/or mitral valve stenosis. Mild to moderate hepatic impairment. Severe renal impairment. 2nd- or 3rd-degree AV block. Significant & acute hypotension. Gingivitis & periodontitis. Avoid concomitant intake of grapefruit juice. Galactose intolerance, Lapp lactase deficiency or glucose-galactose malabsorption. May affect ability to drive or operate machinery.
Adverse Reactions
Flushing, headache, tinnitus. Peripheral edema.
Drug Interactions
Enhanced antihypertensive effect w/ other antihypertensives & TCAs. Increased plasma levels w/ cimetidine, azole antifungals (eg, itraconazole, ketoconazole), macrolides (eg, erythromycin), telithromycin or HIV-PIs. Increased Cmax & AUC w/ grapefruit juice. Reduced plasma levels w/ carbamazepine, phenytoin, barbiturates (eg, phenobarb) & rifampicin. Enhanced preexisting hypokalemia w/ diuretics & antihypertensive effect w/ hydrochlorothiazide. Increased Cmax of cyclosporine & blood levels of digoxin.
